Typically units run three days a week (M-W-F) and do some sort of MSE twice a week (Tu-Th). The biggest issue I see is leaders (not just LTs) do not understand how to vary intensity. A common one is every run is a fast run. Leaders push their Soldiers to always run fast or make every run a competition. When I designed a new run program for my company, our average Soldier improved by between 40 seconds to 1 minute. I did not increase how far the Soldiers ran or how many days they ran. That stayed the same or decreased. However, I changed the format of the runs. Monday was a speed day (shorter sprints of some sort). Wednesday was a mid distance day (longer speed repeats, fartleks, hill workout, etc). The intensity on these days varied. Friday was a longer run, but at a slow pace. Their target speed was 1-2 minutes per mile slower than their race pace mile (ex, if they ran a 12 min two mile, they would run the Friday runs no faster than 7-8 minutes per mile). I would make some Fridays slower (around 2 minutes slower per mile) and some closer to 1 minute per mile. The same concept is true when it comes to MSE training.
